 ss of chronic disease self-management education

 ffiche du séminaire\nRésumé :Patients with chronic conditions have com
 plex and extensive health and social care needs that are not well served b
 y top down models of care.\nWith the number of chronic patients increasin
 g\, the quality of their care largely depends on patient empowerment throu
 gh self-management education\, aimed at increasing the patients’ capacit
 y to manage their own disease and care process.\nHowever\, while self-man
 agement education programs exist for most chronic conditions\, their effec
 tiveness varies widely. Factors that impact on the effectiveness of self-m
 anagement education programs include the program characteristics and forma
 t\, the patient characteristics including multimorbidity and health litera
 cy\, implementation fidelity and adaptation\, and organisational character
 istics.\nThe presentation will draw on the results of the EU-funded DIABE
 TES LITERACY study and related studies to discuss the critical success fac
 tors of chronic disease self-management education and offers suggestions f
 or improvement. It will be argued that in planning for the future\, health
 care policies and models of care are required that cater for the complex n
 eeds of patients and deliver coordinated care that is patient-centred\, em
 power patients to self-manage\, and focus on multidisciplinary teamwork an
 d shared decision-making.\nConférencier :Stephan Van den Broucke is Prof
 essor of Public Health Psychology at the Université Catholique de Louvain
 \, Belgium.\nHis research interest include health promotion planning and 
 evaluation\, health behaviour change\, public health capacity building\, a
 nd health literacy.\nProfessor Van den Broucke has participated in severa
 l large scale international projects\, including the European Health Liter
 acy Survey (HLS-EU)\, Diabetes Literacy\, and IC-Health\, and published wi
 dely. He is an Executive Board Member of the International Union for Healt
 h Promotion and Education (UHPE)\, Advisory Board member of the Asian Heal
 th Literacy Association\, President of the Well Done Health Literacy Award
